Parent/Athlete Information

Athletic Fee
All athletic team members will be expected to pay an athletic fee of $50.00 to participate in IACS athletics.  This fee must be paid for each athletic season of participation.  Please make checks payable to Innovation Academy, or IACS.
 
Required Athletic Paperwork
In order to participate as a member of any athletic team at Innovation Academy, every student-athlete must turn in all required paperwork BEFORE the first day of the respective team's tryouts.  Any student who does not have all required documents on file will not be allowed to try out for that team, no exceptions.  For high school sports' start dates, refer to the High School Athletics tab.  For middle school dates, please refer to Middle School Athletics.  Each prospective student-athlete must turn in the following:
1. A current physical form (valid for 13 months from date of exam)
2. A signed copy of the Student Health Information/Parent Permission form (valid for one (1) school year) - can be found on the Medical Information link
3. A signed copy of the EAA Medical Treatment and Liability Release Form (MS Athletes ONLY) - can be found on Medical Information link
4. A copy (NOT the original) of the student's Birth Certificate (MS Athletes ONLY)
5. $50.00 Athletic Fee (checks payable to "Innovation Academy", or "IACS".  In memo section please write "Athletics")

Athletic Equipment Return
All student-athletes who play on any IACS athletic team MUST return all team equipment, or pay for any lost/stolen equipment BEFORE he/she will be eligible to continue to participate in athletics.  Any athlete who does not turn in all of his/her equipment may also have his/her grades withheld until such time all equipment is returned or paid for.  Any player who plays on multiple teams throughout the year must return, or pay for any lost/stolen equipment from the previous athletic season before he/she will be allowed to tryout for the next team.  All athletes will be held responsible for any damaged*, lost, or stolen equipment while it is in their care.
* - players will not be held accountable for damage that occurs during team practice/games.
