Medical Information

Medical Clearance
Each student athlete must have a current physical form on file with the school's Health Office AND a signed Sports Health Information/Parental Permission form on file with the Athletic Director.  Both forms must be on file BEFORE the student-athlete will be allowed to participate.  If the paperwork is not on file before the date of the first try-out/practice, it will be the coach's choice whether to allow that student-athlete to participate.  DO NOT WAIT!!!  Get your paperwork done as soon as possible. 
Physical Forms:
Physicals are valid for a period of 13 months from the appointment date.  If the physical expires before the end of the current athletic season the physical will be honored until the end of that season, however it must be renewed before the student athlete will be permitted to participate in another season.
 
Sports Health Information/Parental Permission Forms:
Student Health Info/P.P. Forms are only valid for the current school year.  Athletes participating in multiple athletic seasons will only need to turn in one (1) form for the year.  Signed (by parent) forms must be submitted to the Athletic Director by each student-athlete before the he/she will be allowed to tryout.  Click the link at the bottom of this page for a copy of the Student Health Info/P.P. form.  Blank forms are also available from the Athletic Director.
